Insurance, Covered and Non-Covered Charges

The Franklin Eye Center participates in most major health care plans. We will gladly submit your claims for you, provided you supply us with your most recent insurance card at the time of your visit.

Your examination charges are usually covered by your health insurance if your visit can be coded with a medical diagnosis. This coverage can include diseases and conditions affecting your eyes, such as cataracts, macular degeneration, dry eyes, and glaucoma. Your health insurance may also cover ocular examinations associated with systemic health problems such as diabetes, hypertension, arthritis, thyroid disease, and headaches.

Dr. Wohl is contracted with most major vision insurance plans. Even if you do not have coverage for your yearly routine eye exam, you will find our self-pay rates highly competitive and an amazing value for the level of service we provide.

Important Information About Our Fees

At the Franklin Eye Center, we will check your insurance benefits as a service to you. We want you to be aware of several issues concerning insurance coverage.

Refraction

The determination of corrective lenses for your eyes is a service that may not be covered by your insurance plan. You may be asked to pay a separate fee for this service.

Contact Lens Fitting

This service is not always covered by insurance plans. Each year, if fitting of contacts is needed, a one time fee will cover all necessary visits.

Insurance Referrals

Please remember to call your primary doctor’s office for a referral at least 3 days prior to your appointment with us if required by your insurance plan.

Payment Options

Co-pays and payments for services not covered by your insurance are due at the time of service. Visa, MasterCard, American Express, Discover Card, and personal checks are gladly accepted.

Laser Vision Correction can be financed through CareCredit or ChaseHealthAdvance, which offers both long-term, low payment and shorter-term, no-interest options.
